What is Mater at Home?
Mater at Home offers a professional healthcare service to clients in their own home. Mater at Home delivers a range of health professional services to clients residing in the Brisbane metropolitan area, including the Bayside area.
Mater at Home also maintains close working relationships with hospitals, community health care providers and general practitioners to ensure you receive seamless health care from hospital to home.
What can Mater at Home help you with?
Our dietitians can help with unexpected weight loss, loss of appetite, inability to lose weight, managing diabetes or IBS and access to meal services.
Our occupational therapists can help with improving and adapting daily tasks to help you stay independent at home, home modifications tailored to your needs (rails, ramps, etc) and prescribing equipment—from eating utensils to power drive wheelchairs.
Our physiotherapists can help with difficulty walking or moving, problems with pain and stiffness, breathing difficulties and lung conditions problems with balance or falls.
Our psychologists can help with management of stress, anxiety and depression, support with adjusting to illness or disease, management of pain and sleep difficulties and strategies and skills to increase quality of life.
Our social workers can help with fears and worries associated with illness, disability or plans for future care needs, accessing other services to assist you at home and support for carers.
Our speech pathologists can help with coughing or choking when you eat or drink, any problems with swallowing and communication difficulties.
Who can use Mater at Home?
Mater at Home services are available for anyone living in our service area who needs assistance with any of the issues outlined in this brochure.
Where does Mater at Home provide services?
Mater at Home provides services primarily in the QEII Hospital and Bayside Health Services Districts.
Mater at Home also provides services in the Brisbane metropolitan area (suburbs within a 30 kilometre radius of Brisbane,plus Ipswich and some Logan areas) under the following funding arrangements: Post Acute Care (PAC), Transition Care (TCP), Department of Veterans Affairs (DVA), Extended Primary Care/Chronic Disease (EPC/CDM) and private patients.
How do you access Mater at Home services?
Mater at Home accepts referrals from many sources, including:
- community health centres
- home-based service providers
- other health professionals
- family members
How much does it cost?
Most clients will be eligible for Mater at Home at no charge; please discuss this with our friendly staff. Private services are also available for a fee.
Links to other services
Mater at Home will maintain close working relationships with your hospital, other health care providers and your general practitioner.
Mater at Home can, and will, refer on to other service providers if required or will advise you on how best to access another service provider if needed.
