Healthy aging

We are passionate about assisting people to maximise their independence and enjoyment within the home and improving quality of life for residents in aged care facilities. People often seek referral for therapy to improve movement and strength after illness or surgery; falls prevention assistance and modifications to homes to increase safety; help to cope with cognitive decline and
carer stress; advice for future planning and referral to other community services.


A quality team of health professionals experienced in providing nursing and allied health services for infants, children, adolescents and their families within the comfort of their home or other community settings such as schools. Our team work closely with the family, carers, referrers, GPs, treating hospital and other community services to provide comprehensive care.

Our team of experienced health professionals are committed to supporting individuals to thrive and grow in the community in addition to supporting families with the capacity to care for their family member living with a disability.
Mater at Home is a recognised service provider with:

  • Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA).

We work closely with the family, carers, referrers, GPs, treating hospital and other community services involved to provide comprehensive care.

Palliative care

Mater at Home provides a team of experienced health professionals to help support patients and their families who have a terminal illness. The team are able to assist the patient by making them comfortable with an aim of maintaining their quality of life at home. Our team works closely with the family, carers, referrers, GPs, treating hospital and other community services to support physical, psychological and social needs.

Post acute care

This service is available for patients who may need extra care following discharge from hospital to assist in their recovery while at home. Post acute care is available for all age groups and for patients from any Queensland hospital.

Hospital in the Home (HITH)

The Hospital in the Home (HITH) program provides eligible patients care for acute conditions in their homes as an alternative to being treated in hospital. This gives patients greater independence, choice and comfort. HITH is available to all age groups and patient eligibility is determined by the patient’s authorised medical practitioner.


Mater at Home offers video conferencing as an innovative solution to geographic challenges and
to increase our flexibility in meeting individual needs. In many cases our experienced allied health
professionals can assess and advise individuals remotely at a time that is convenient.

Mater Aged Placement Service (MAPS)

MAPS is an aged care placement and advisory service, which helps those requiring assistance with
the transition to an aged care facility. Our experienced social workers have expertise across the health and aged care systems, and will work with you to simplify the process. Our staff understand the admission process of aged care facilities and have extensive experience in
liaising with older people and their families.

Who can use this service?

Referrals can be received from individuals, their family members or other health professionals acting on their behalf. Home visits are available within the Brisbane metropolitan area. Telehealth is available for anyone living outside of this area. Please call the office for further information.

How much does it cost?

Mater at Home services are available at affordable and competitive rates. Please contact us for information on current rates and plans.

Mater at Home also receives funding from various funding schemes. Subsidies may be available for eligible clients, please contact us
for further information on funding eligibility.
